Prior to your appointment
About a week before your initial visit, you will receive an email from us with paperwork attached.
Please email us the following materials prior to your appointment:
- New patient registration paperwork
- Picture of the front and back of your insurance card (if applicable)

If you are waiting outside of the building for your appointment, please call/text 206-518-9022 and request to unlock the front door.
