Thank you for choosing our clinic to help with your physical goals. The information here will help to make your first visit with us a pleasant and positive experience.

Arriving

Two parking spots are available behind the building. You can access this parking lot through 46th St. If neither of those spots are available, there is free 2-hour parking on the surrounding streets.

We suggest wearing comfortable clothing that will allow you to move about. We no longer require masks to enter our clinic, but they will be available in our waiting area if you choose to wear one. If you would like your provider to wear a mask, just let us know!

Checking In 

Our Front Desk staff will need the following:

• Photo ID
• Insurance card
• Payment card to save on file
• Registration paperwork 

Velocity Physiotherapy is required to protect the privacy of your personal health information. For more information, click here. 

We require 24-hours notice for any cancellations.

Appointments that are not cancelled at least 24-hours in advance will result in a $50 late-cancellation fee.

We understand that life happens and it can be hard to keep track of your busy schedule! Therefore, we will waive the first late-cancellation fee.

Cancellations due to illness or family emergency will not result in a late-cancellation fee.

Appointments that result in a no-show (i.e. no explanation provided before the appointment start time) will result in a $50 no-show fee. All future appointments will be cancelled until this fee is paid.

Starting January 1, 2023, we will ask every patient to provide a payment card to save on file with our clinic, regardless of insurance type of payment history.

Here is what you need to know:

Once your card is on file, in-person payments will no longer be necessary.

If you have a copay, it will be automatically charged on the following business day of the date of service.

You will receive monthly statements in the mail during the second week of each month. We ask that you look over these statements to make sure everything looks right. 

On the last business day of the month, your payment card will be automatically charged the amount provided on your statement.

You will receive an emailed receipt for any transaction made through your card on file.

Velocity Physiotherapy uses an app-based home exercise program called Theracentric. 

Through the app, your therapist can video record the exercises you do during your in-person session to then upload to your private Theracentric account. You'll receive reminders to do your exercises and be able to watch yourself doing them correctly as instructed by your therapist.

As our patient, you will automatically be given access to this app. It is completely up to you whether or not you want to use it.

Theracentric is HIPAA-compliant, so only you would be able to see your exercise videos.

There is no cost for our patients to use Theracentric.
