Arriving

Two parking spots are available behind the building. You can access this parking lot through 46th St. If neither of those spots are available, there is free 2-hour parking on the surrounding streets.

We suggest wearing comfortable clothing that will allow you to move about. Anyone entering our clinic is required to wear a face mask to reduce and prevent the transmission of COVID-19.

Checking In

Our Front Desk staff will need the following:

Photo ID
Insurance card
Payment card to save on file
Registration paperwork
