Please allow yourself time to find parking and check-in with our Front Desk.

Two parking spots are available behind the building. You can access this parking lot through 46th St. If neither of those spots are available, there is free 2-hour parking on the surrounding streets.

We suggest wearing comfortable clothing that will allow you to move about. Anyone entering our clinic is required to wear a face mask to reduce and prevent the transmission of COVID-19.

Our Front Desk staff will need to scan your paperwork, photo ID, and insurance card upon arrival. Please also bring the credit card that you would like to save on file. This will only be necessary for your initial visit.
