Yes. Since we are considered a healthcare facility, we require anyone entering our clinic to wear a face mask in order to reduce the spread of COVID-19.

All patients of Velocity Physiotherapy are required to save a card on file. Once your card is saved on file, any remaining outstanding balance will be automatically charged once a month.

If you are receiving insurance coverage for our services, please note that the charges shown on your statement are provided to us by your insurance. Patients are responsible for contacting their insurance regarding any statement concerns.

We require 24-hours notice for all cancellations. Appointments not kept or cancelled less than 24-hours before your scheduled appointment time will result in a $50 late-cancellation/no-show fee. Rescheduling will not be permitted until this fee is paid.


We understand that life happens and things can come up! A first time late-cancellation/no-show will be waived of this fee.

Physical Therapy is for rehabilitation or treatment of a specific injury or area of concern; an ongoing medical issue that is affecting movement and daily life; and preventing disability. Most insurances will cover this service if it is deemed as medically necessary (i.e. there is a diagnosis).


If you are a dancer or athlete hoping to improve your skills, we suggest scheduling a Comprehensive Performance Assessment, which examines the entire body rather than one specific area. This service is not covered by insurance.

We are in-network with most insurances except:

  • Apple Care
  • Medicaid
  • Molina
  • United Healthcare - Effective Jan. 1st, 2023

For any other types of appointments, please call, text, or email us to book your appointment. If you are a new patient, please have your insurance card on hand.

There are two parking spots behind the building that are available on a first-come, first-serve basis. The entrance into this parking lot is through 46th Street.


If both parking spots are taken, there is 2-hour street parking available on the surrounding streets.
